Apple Vision Pro: First cataract surgery worldwide successful

A New York eye surgeon is performing a procedure using Apple's mixed reality headset that was previously considered impossible.

The Apple Vision Pro has a tough time in the mass market – that's no secret. But in specialized professional fields, the headset is increasingly becoming a serious tool. Now a new milestone has been reached: For the first time, a cataract operation – one of the most common surgical procedures worldwide – has been performed using the Vision Pro. What sounds like an isolated case could actually be the beginning of a whole new era in eye surgery.

Who performed the procedure

Dr. Eric Rosenberg, an eye surgeon at SightMD in New York, is responsible for the world's first procedure of this kind. He performed the initial procedure in October 2025 and has since completed several hundred more operations using the Vision Pro. This is made possible by a specialized platform called ScopeXR, which Rosenberg himself helped develop.

ScopeXR transmits the live image from a digital 3D surgical microscope directly to the Vision Pro. The surgeon sees the surgical field in stereoscopic 3D and can simultaneously display preoperative diagnostic data in their field of view. This is a function that would not be possible in traditional operating rooms.

What makes the platform special

The real advantage of ScopeXR lies in its real-time collaboration. Experienced surgeons can participate virtually in operations and see exactly what the operating surgeon sees on-site. This opens up entirely new possibilities:

  • New employees receive direct live support from experienced colleagues.
  • In case of unexpected complications, specialists can be called in from around the world.
  • Operating rooms in remote regions can access top expertise in real time.
  • Training and knowledge transfer are spatially decoupled.

Rosenberg summarizes it this way: The technology democratizes access to surgical expertise – and can help save the eyesight of patients who would otherwise not receive care.

Vision Pro is gaining ground in the professional sector

The success in eye surgery reflects a clear trend: Apple is increasingly positioning the Vision Pro as a tool for specialists, not a mass-market product. Several pilot projects are already underway in the medical field. The headset is also being used in pilot training, architecture, and industrial design. The high starting price of $3,499 is more easily justified in these professional applications because the costs can be weighed against a clear economic or medical benefit.

The Studio Display XDR recently received FDA approval for radiological applications. Apple has thus deliberately opened the door wide to medical applications – a profitable and rapidly growing market.

What the future holds for Vision Pro

Despite its success in the professional sector, the Vision Pro remains highly competitive in the consumer market. Reports indicate that Apple is not currently developing further Vision headsets on a large scale – instead, the focus is on lightweight smart glasses. These are intended to better meet the everyday needs of many users than a bulky headset. In October 2025, Apple did introduce a hardware refresh: the Vision Pro with the M5 chip, the first true update since its original release.

It's also interesting that Apple is simultaneously working on a modular future for the headset. As a recent patent for the modular and upgradeable Vision Pro shows, the next generation could be significantly more flexible – possibly with interchangeable components for professional applications such as eye surgery. There's also a lot happening in the software ecosystem: Steam Link is coming natively to the Vision Pro, which should make the platform more attractive for gaming. Overall, it's clear that Apple hasn't given up on the headset, but is clearly focusing on two main areas – specialized professional workflows and targeted software extensions.

What this means for medicine

From our perspective, the successful application of this technology in cataract surgery is a significant step forward. Cataract operations are among the most frequently performed procedures worldwide – in Germany alone, according to the Professional Association of Ophthalmologists, around 700,000 to 800,000 cataract surgeries are performed annually. If such routine procedures can be performed even more precisely and ergonomically in the future using mixed-reality headsets, with live support from experienced colleagues worldwide, patients will ultimately benefit most.

The Vision Pro may never become a mass-market product in the living room. In the operating room, however, it could be a real game-changer. This demonstrates once again that the evaluation of Apple's headset cannot be based solely on sales figures – but rather on where the technology actually makes a difference.
